Become a Patient - Inpatient Rehabilitation

Step 1 – Referral: Ask your physician, social worker, or case manager to send a referral on your behalf to Sheltering Arms Institute. Your referral should include the following information:

• General information, including name, date of birth, address, emergency contact, and insurance information
• History and physical
• Medication administration list
• Most recent physician notes
• Therapy evaluation(s) and most recent therapy treatment notes

Step 2 – Determining Appropriateness and Eligibility for Inpatient Rehab: A clinical referral liaison will be assigned to your case to answer any questions you may have, evaluate your medical chart, and ensure you meet the criteria for intensive inpatient rehabilitation required by Centers for Medicare & Medicaid Services. The liaison will also assess your insurance coverage and/or financial needs.

Step 3 – Insurance Authorization: After the liaison has gathered all necessary information and determined that you meet all admission criteria, s/he will work to obtain financial approval via your insurance company or our financial assistance program. 

Step 4 – Admission: Your liaison will alert you if and when your case has been approved. S/he will collaborate with your current care team to determine your anticipated date of admission based on your medical readiness.
