Become a Patient - Outpatient Therapy

Sheltering Arms Institute offers a full range of outpatient therapy custom designed for you. Outpatient therapy can help you continue your progress after graduation from inpatient rehab or can be the starting point of recovery for less severe injuries or illnesses. We provide a seamless transition in your recovery journey by communicating with your care team prior to beginning outpatient therapy.

Outpatient Therapy

To become a patient at one of our outpatient centers, please follow the steps below: 

  • Step 1: Browse our list of conditions treated and services offered to ensure we offer care to meet your needs. 
  • Step 2: Check our list of accepted insurance plans to see if your carrier is listed and call the phone number or visit the website on your insurance card to confirm coverage.
  • Step 3: To make an appointment, call (804) 764-1000 Monday through Friday between the hours of 7:30 a.m. – 5 p.m. to speak with a representative, or submit our online contact form. If you have seen a healthcare provider for the injury or illness for which you are seeking therapy services, ask your provider to submit a referral on your behalf to Sheltering Arms Institute.

Preparing for Your First Visit

What to Expect

  • After scheduling your appointment, review your Rights & Responsibilities and Rights & Protections Against Surprise Medical Bills as a patient.
  • 72 hours prior to your appointment, you may receive a text message and/or email* prompting you to complete a brief questionnaire and initial assessment.
  • 48 hours prior to your appointment, you may receive a text message and/or email* reminding you of your appointment date, time, and location along with a prompt to complete pre-check paperwork for your appointment, including insurance information and consent forms.
  • The day of your appointment, please arrive 15 minutes early. Please check in using our kiosk and be sure to bring the following items with you:
    • A valid photo ID
    • All medical insurance cards, including Medicare and Medicaid
    • Your prescription for therapy from your doctor (if applicable)
    • A printed and completed copy of the outpatient history questionnaire (if you’d like to complete this ahead of your appointment)
    • Your insurance referral or authorization (if your policy requires you to obtain one prior to seeing a physical rehab provider)
    • Your claim number, insurance carrier’s billing address, and job description (if your visit is being covered by Workers’ Compensation)
    • Your claim address and number; at-fault insurance details; adjuster’s name, phone and fax numbers as well as your attorney’s name, address, phone and fax numbers if you have an attorney (if your visit is due to a motor vehicle accident)
  • Throughout your physical rehab plan of care with Sheltering Arms Institute, you may also receive text message and/or email* appointment reminders prior to each visit as well as ongoing assessments to measure your progress. Your clinician will also prescribe you a customized home exercise program to help you achieve your best outcomes; this can be accessed via the Sheltering Arms Institute app.

Becoming a Patient FAQs

What to expect during outpatient therapy at Sheltering Arms Institute

When you first arrive for your evaluation, please check in at our kiosk before proceeding to our front desk to complete your registration. There is also some initial paperwork to complete at your first appointment, so please arrive at least 15 minutes before your scheduled appointment time.

During your first therapy session, your clinician will complete an assessment, discuss your goals, and collaborate with you to develop your therapy plan. We will start by recommending a certain number of visits and then reassess regularly, about once a month. This frequency varies but typically ranges from 1-3 times a week. This appointment frequency can be an adjustment if you just completed inpatient rehab where you may have participated in more sessions per day or week. If insurance allows, you are making progress, and there are more functional goals to address, we may extend the plan of care at the time of your reassessment.

At times, we may need caregivers/family members to participate in part of your session, such as during your initial evaluation or for caregiver training. However, given space constraints and patient privacy, patients typically participate in sessions independently. Your caregiver/family member is welcome to wait in our lobby during your appointment.

Caregivers/family members are welcome to ask questions and provide feedback regarding your progress or function during your appointment. At the end of your session, your therapist will transition to their next patient’s appointment, so please ask questions or provide feedback during your session to respect time limitations.

Your therapist will issue a home exercise program aligned with your goals to work on when you are not attending therapy appointments. You will receive an email and/or text message prompting you to download the Sheltering Arms Institute app. This app will help you prioritize your home exercise program, giving you the best chance of achieving your goals. Through the app, you can play videos demonstrating how to perform each exercise and how often.

If something is unclear in your home exercise program, or you have trouble accessing it through the app, please ask your therapist for help as soon as possible. Paper copies of your home exercise program can also be provided upon request.
