After Therapy Graduation: What’s Next on Your Rehab Journey

When: Wednesday, November 20th from 1-2 p.m.

Where: In person at Sheltering Arms Institute’s Main Campus (2000 Wilkes Ridge Dr. Henrico, VA 23233) or virtually via Microsoft Teams

Description: This presentation will entail opportunities that are offered following inpatient rehabilitation discharge. It will discuss ways to increase intensity and frequency of rehabilitation through outpatient physical therapy and fitness and wellness services. It will help you understand and navigate through fitness and wellness services to discover which program will provide you the most appropriate benefits to help achieve therapy and personal goals following therapy graduation. Lastly, this presentation will highlight if and when it is appropriate to return to physical therapy.

Learning Objectives:

1. 1. Distinguishing appropriate pathway to receiving physical therapy following inpatient rehab discharge
   2. Justifying the need for an intensive therapy plan
   3. Understanding what discharge from physical therapy means
   4. Comparing and differentiating services that are offered through Sheltering Arms Community Programs (fitness, recreation, and wellness services)
   5. Estimating and evaluating when to return to physical therapy
